Here are two questionnaires in my work and learn research project in PDF format. In practice, of course, these are in web survey format using Qualtrics.
Keep in mind that the employer's survey uses branching, and that most respondents will only see one section of this survey depending on whether they have students in internships, co-ops, apprenticeships, practicums, or federal work study positions. In reality, it is much shorter than it looks with the full branch possibilities seen in the PDF.
Here is some suggested language you can use in an email to your employer partners:
I've recently become aware of a research project that looks at characteristics of programs in which college students are employed while they are taking classes. We all want to support our working learners, and make the programs we have better, and this project seeks to uncover information that will help us do that. The project uses questionnaires for employers and for people on the education side. I've completed the one for our organization, and hope that you would consider taking the one for employers.
This research is being conducted by a leading researcher in the field of higher education, John Pryor, with support from the ACT Center for Equity in Learning. They assure us that all results are confidential and will only be reported in group fashion, not by individual school or organization. They will be available in a free report later in the year.
